Vaccines in bold type are required school immunizations.

* May be given as early as 12 months of age.  Not needed if the child has already had chickenpox disease.

** Any child who has reached his or her fifth birthday shall not be required to provide proof of immunization with pneumococcal conjugate vaccine.

The LCDPH offers immunizations through the "Vaccines for Children" (VFC) program to children under the age of 19 that are Medicaid eligible or whose insurance doesn't cover vaccines. An administrative fee of $23.75 per vaccine is charged.  Children whose insurance covers vaccines are encouraged to contact their primary healthcare provider.

The health department also offers adult immunizations, including flu, meningococcal, pneumonia, and tetanus.  Some foreign travel immunizations, such as Hepatitis A, Hepatitis B, and Typhoid are also available.  For additional information, please call 217-735-2317 or email us at info@lcdph.org.

Additional immunizations may be recommended or needed if your child will be traveling to another country or has certain high risk medical conditions.  Please consult with your physician or reference the CDC website.
 

We have two forms that must be filled out by the legal guardian, the Vaccine Administration Record and the Clinic Contradiction Checklist.  If these are signed by the legal guardian and notarized, relatives or friends of the family may bring children in for immunizations.  Click here for FORMS.
